Remote Patient Monitoring

What is Remote Patient Monitoring (RPM)?

MidAtlantic Nurse Advocacy Group is care management practice that provides Remote Patient Monitoring for patients who have chronic conditions not limited to Asthma, COPD, Diabetes, Heart Failure, Hypertension and Sleep Apnea. We monitor your condition by way of devices such as blood pressure machine, blood glucose monitor, digital scale and pulse oximeter.

Healthcare is changing and with RPM, technology and automatic data transmission can be utilized to monitor your clinical status while you are in the comfort of your own home. Data is sent directly from you to your nursing team and then your nurse can review and analyze vital sign data to help keep you out of the hospital. Remote patient monitoring is an efficient way for patients and doctors to be connected with the need for travel.

Manage Your Chronic Condition with Remote

Patient Monitoring

Frequent trips to the emergency room and unplanned hospital admissions often weigh heavily on patients with chronic conditions. But what if monitoring and managing your condition from home was as simple as taking your blood pressure, stepping on a scale, checking your blood glucose level or checking your oxygen level. Once data are collected it is automatically sent to your own personal team of nurses, who will evaluate your results for any signs of illness or change in condition. Your doctor is also made aware so adjustments in treatment plan can be made as needed. This could drastically decrease ER visits and hospital admissions and improve your quality of life.

We provide peace of mind

Even though you are at home, take comfort in knowing, your personal nurse is monitoring your clinical status to help keep you out of the hospital.

High Quality Service

We are your personal team of nurses and provide education, encourage self-management and interactively communicate with you. We aim for optimal patient outcomes.
